An operating hazard assessment, often called an OHA, is a systematic review of the specific hazards introduced when a transportation system is operated day-to-day. It focuses on how vehicles, infrastructure, procedures, personnel, and the environment interact during revenue service, maintenance, and emergency conditions.

General risk assessments take a wider organizational view, looking at threats, finances, and enterprise risk. An OHA is narrower and more technical, producing a hazard log with severity and probability ratings for each scenario. The two tools complement each other, with the OHA feeding specific operational hazards into the broader safety risk management process.

OHAs are commonly required during safety certification of new rail lines, stations, vehicles, and signaling systems before passenger service begins. They are also triggered by significant changes to existing operations, such as new rolling stock, revised operating rules, route extensions, or the introduction of new technology like autonomous shuttles.

Under FTA’s Safety Management System framework, ongoing hazard identification is expected as part of a transit agency’s Agency Safety Plan. Most mature agencies also conduct targeted OHAs after serious incidents to identify whether similar hazards exist elsewhere in the system.

Common methods include Preliminary Hazard Analysis (PHA), What-If analysis, Failure Mode and Effects Analysis (FMEA), and Hazard and Operability studies borrowed from the process industries. Many transit agencies align their approach with Military Standard (MIL-STD) 882 or equivalent system safety standards.

The right methodology depends on the phase of the project and the complexity of the system. Early design work often uses broader techniques like PHA, while detailed OHAs for specific subsystems such as signaling or traction power use more rigorous FMEA or fault-tree analysis. The goal is always a documented hazard log that supports decisions, not a theoretical exercise.

Effective assessments require input from operations supervisors, maintenance leads, safety staff, engineering, training, and frontline personnel who actually perform the work. External perspectives from signal engineers, vehicle manufacturers, and emergency responders add value for complex projects.

Involving frontline workers is especially important because they routinely see the workarounds and edge cases that designers and planners miss. A workshop-style OHA with the right people in the room typically identifies more hazards in two days than a desktop review does in two weeks.
